- The form can be downloaded at the bottom of this page
- Important: Your claim cannot be processed without this completed form
- Please upload the completed DOR form when submitting your request below
If your order arrived damaged
- Provide clear photos of the damaged items
- Include photos of the packaging showing any external damage
- These photos are required to process your claim
If items are missing
- List which items are missing from your order
- Photos can help but are not required for missing items
